Applying for a grant

Requests or proposals must include:

  • Clear identification of the recipient charity, including at least one of the following:
  • Executed IRS Form W-9 (Request for Taxpayer Identification Number and Certification).
  • Copy of tax-exemption determination letter from the IRS.
  • Copy of the most recent annual report (IRS Form 990 or 990-EZ).
  • Letter from the charity specifying the basis of its tax-exempt status, if not a 501(c)(3) public charity (e.g., a state university is tax-exempt, but not under Section 501(c)(3)).
  • Taxpayer Identification Number.
  • Request on the organization's letterhead detailing the amount of funds sought.
  • Program objectives.
  • Description of intended use of requested funds and which of the Foundation's philanthropic objectives would be achieved.
  • Description of all benefits offered for the gift (e.g., inclusion on the list of donors, name plaque, promotional materials, etc.).
  • Overall budget of the program.
  • Description of how the recipient organization will measure success.
